I give permission for my child’s participation in Vacation Bible School (hereafter VBS)  to be held at High Pointe Church August 5th-8th, and represent and agree that: 1. I understand that participants of VBS will be involved in activities such as scheduled recreation and games, team time with leaders, outdoor activities, worship, and Bible teaching. 2. I am aware of the potential hazards and risks to my child and property associated with participating in VBS, such hazards and risks including, but not being limited to illness, injury or death by accident. I accept my child’s participation in VBS with full awareness of these risks and I agree on behalf of my child that neither I nor they will make a claim or bring legal proceedings against High Pointe Church in connection with their activities or participation in VBS. With respect to High Pointe Church and its agents, officers, volunteers, directors, and employees, I voluntarily, on my child’s behalf, assume all hazards and risks and any damage to property on behalf of my child, I release High Pointe Church and its agents, officers, volunteers, directors, and employees from any liability that my child may suffer as a result of participation in VBS. 3. I attest and certify that my child has no medical conditions that would prevent them from participating in VBS. 4. I am aware of the hazards and risks to my child associated with participation in VBS, as described above. I further understand that High Pointe Church does not have any insurance coverage that would apply in the event of illness, injury or death, or damage to my property that may occur during or as a result of my child’s participation in VBS, and that if I desire insurance coverage I am responsible for obtaining and paying for such insurance for my child. 5. I expressly agree that this assumption of risk agreement is intended to be as broad and inclusive as permitted by law. I further state that I have carefully read the foregoing assumption of risk and understand its contents, and I voluntarily sign this release as my own free act on behalf of myself and each child of mine. This is a legal document and I understand that I have the opportunity to consult with an attorney before signing it.

In case of my unconsciousness, or my inability to authorize medical treatment for any situation in which my child has a need for medical treatment resulting from illness, injury, or an accident which requires medical attention, I give my permission to High Pointe Church and its representatives, and all attending health care professionals (defined as including, but not limited to registered nurses, licensed practicing nurses, physicians' assistants, doctors and paramedics) to provide medical treatment, to hospitalize, anesthetize, or perform surgery on my child as they in their judgment deem advisable. I, the undersigned, do release, acquit, discharge and covenant to hold harmless High Pointe Church and its representatives from all actions, damages or liabilities arising out of the treatment of any illness, injury, or accident incurred during my child’s participation in the 2025 KidZone Vacation Bible School sponsored by High Pointe Church. It is the intention of this release that High Pointe Church and its representatives incur no liability whatsoever while attempting to meet any medical needs that my child may have during VBS.
